ILLUMINATION FOUNDATION Lead Care Manager (ECM) in Fullerton, California

Illumination Foundation Medical Group delivers comprehensive care by a team of providers that include an M.D., psychiatrist, advanced practice nurse, counselors, behavioral health therapists, and social workers. The mission and guiding principles of Illumination Foundation are supported through the work of the talented individuals providing medically related services at Illumination Foundation Medical Group (IFMG). Working in conjunction with Illumination Foundation providers, IFMG removes barriers to healthcare by offering outpatient primary care services to clients who are currently occupants located at Illumination Foundation sites throughout various counties in Southern California.

Job Description

Lead Care Manager is an interdisciplinary, high touch, client centered role, which assumes primary responsibility for coordinating members needs through a variety of engagement modalities, requiring adherence to and application of evidence-based practices, knowledge of client and service barriers as well as social determinants of health, and deliverance and coordination of services. Lead Care managers will function as a community facing position wherein they will seek out populations of focus and identified by providers and engage, assess, enroll, and advocate for them on a routine basis.

The pay range for this position is $23.00 - $26.00 per hour, depending on experience.

Responsibilities

Client Care:

  • Lead Care Managers will be responsible for identifying and addressing all qualifying

members’ needs telephonically and in person, in various settings and in accordance with

social determinants of health in the following ways:

  • Assembly, implementation, and fulfillment of the comprehensive assessment and care

management plan

  • Identification of barriers to treatment and medication management, as well as advocating for and promoting positive, goal-oriented health outcomes.

  • Assist with navigation of physical health, behavioral health, developmental, and social services.

  • Monitoring and supporting treatment adherence.

  • Assisting in attaining goals as established by the client and their care team.

  • Coordination of all appointments including accompaniment, when necessary, transportation, as well as assistance in navigating various healthcare services.

  • Monitoring referrals and following up to ensure services are accessed and loops are closed

  • Successfully facilitate comprehensive transitional care amongst various facilities and providers within the designated county of operation.

  • Maintaining fluid communication and collaboration with agents of assigned client health networks as needed.

  • Assisting in the establishment and grounding of desired familial support systems belonging to the members served.

  • Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services.

  • Engage eligible members.

  • Offer services where the member lives, seeks care, or finds most easily accessible and within CalOptima Health guidelines

  • Advocate on behalf of members with health care professionals.

  • Use motivational interviewing, trauma- informed care, and harm-reduction approaches.

  • Coordinate with hospital staff on discharge plan

  • Accompany members to office visits, as needed and according to CalOptima Health guidelines.

  • Monitor treatment adherence (including medication).

  • Provide health promotion and self- management training.

    Documentation:

  • Lead Care Managers will be responsible for appropriately navigating and documenting all engagement frequency and content in Illumination Foundation’s EMR in accordance with standards of practice as required by Illumination Foundation and associative contract entities. These practices include:

  • Incrementally documenting all aspects of a client care plan, including goals met as well as goals forthcoming.

  • Documenting level of engagement such as frequency and duration of encounters.

  • Documenting evaluative client case details which factor into decisions made to referral clients to alternative resources.

  • Documenting obtained client records, such as vitals documents, insurance cards, SSI award letters, etc.

  • Documenting client disengagement as well as engagement.

  • Documenting any other such desirable reporting metrics as outlined and required by contracting entities

    Preferred Experience/Minimum Qualifications

    Required:

  • High School Diploma or equivalent. 

  • 2 years relevant experience.

  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets. 

  • Valid CA Driver’s License and eligibility for company vehicle insurance. 

    Preferred:

  • Bilingual in English and Spanish.

  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams)
